A quick fix before submission is a five-minute task. A correction after a denial or audit finding can take hours and often involves escalation.
This is also where technology can do real work. If rework is clustering around a specific IDG team or a specific location, there's likely a workflow issue worth investigating. If certain documentation types — recertifications, for example — are generating disproportionate corrections, the template or the process probably needs to change.
Most agencies don't have easy visibility into these patterns because rework is tracked informally, if at all. Building even a basic system for categorizing and reviewing rework by type, clinician, and documentation category gives compliance and clinical leadership something concrete to act on.
Reducing documentation rework isn't just an operational win. It's a compliance win. Notes that are complete, consistent, and clinically defensible the first time are notes that hold up in audits.
